New Patients
To register with Datchet Health Centre you must live permanently
within the Practice Area.
Before you can be registered you will
need to attend the surgery in person. Please bring your medical
card if available. Any preference for doctor expressed will be
considered by the practice, but we reserve the right to allocate you to
the list of any of our doctors.
- When you first register with us you will be given a Registration
form to complete.
- You can open and print a copy of our Registration form by
clicking here
- Please complete the form and bring it with you to the Health
Centre with proof of your address.
